A template is not a diet

Searches for diet plan template and template for diet plan want a place to write meals and amounts. This page gives you the fields. It does not tell you what to eat, how much to weigh, or how to treat a condition. If a clinician gave you targets, those targets go at the top of your page in their words. If you do not have targets, a meal planner is the better page.

Columns that stay useful: meal, food, portion, and a short note. A calorie column only helps if you are actually using a number you were given and you will keep logging it. An abandoned calorie grid is not a plan.

How it connects to the week

The meal planner decides the dish. This log records what you ate. They can be the same database with a 'planned' and 'eaten' status, or two pages if looking at the plan and the log together makes you less likely to fill either in. Shopping still happens on the grocery list.

Questions

Can this template replace advice from a doctor or dietitian?
No. It stores meals and portions. Medical and nutrition decisions belong with a qualified clinician who knows your history.
